    is the primary support for herself, Tommy and an older sibling (another sibling is in jail). Tommy is home schooled and receives nursing care from home health agencies. A variety  of specialists are provided by the local government operated CMC clinic who are responsible for follow-up on Tommys care. Continuing follow-up is crucial to Tommys wellbeing and care. Karandikar,  et al (2004) point out that young children who survive Shaken Baby Syndrome, that is, "non-accidental head injury," generally suffer from significant physical, cognitive and behavioral problems (p. 178). This  is the case for Tommy, who suffers from seizures. Karandikar, et al (2004) therefore recommend that regular follow-up should be conducted by a multidisciplinary team that includes a pediatrician, therapists,  school health personnel and ophthalmologic services. They warn that care must be taken not to consider Tommys prognosis as being set, as additional behavioral and cognitive problems can emerge over  time (Karandikar, et al, 2004). The student researching this topic does not indicate whether or not it is Tommys grandmother who is implementing a home schooling program or if  a more qualified instructor is provided by county health services. If Tommy is being home-schooled by his grandmother, an educational assessment by the nurses providing his care may be  in order so that it can be determined if all of the childs educational needs are being met. Aiding disabled children in reaching their full cognitive potential is often a  job that even professionals find difficult and Tommys needs may be better fulfilled in a public school program designed to meet the needs of handicapped children. The nursing diagnosis  in this situation encompasses Tommys family situation, since he is dependent on his family for long-term care.
