    This is a proposal for a public health program designed to address heart disease in persons over the age of 65. The location is Coney Island,  New York, where a well developed community focus already exists. The purpose here is to take full advantage of that well developed focus to provide another point of prevention  of heart disease in the area. The Aggregate and Assessment Data The specific aggregate of interest is that consisting of senior citizens over  the age of 65 with heart disease. Additionally, this aggregate is further limited to those senior citizens with cardiac disease living in the Coney Island area of Brooklyn, New  York. Demographic Data New York transit officials state that the area served by the Church Avenue subway station includes Coney Island. According  to transit authority officials, age distribution for this area is: * 0-21 33.48% * 22-64 61.05% * 65+ 5.47% (Church Avenue Station).  There is great demographic variation in the area, however. The Parish Profile for Brooklyn Heights Grace Church (2003) states that there is great diversity  in the general area, but that the population immediately surrounding the church is rather homogeneous. Nearly 29 percent of Coney Island area residents live below the poverty line, the  median income is $36,000 and the population is highly diverse (Church Avenue Station), but the population within a half-mile radius of Grace Church is overwhelmingly white (69.2 percent); highly educated  (35 percent with graduate school degrees); and more than half (54.5 percent) have annual incomes over $75,000 (Parish Profile, 2003).
