regarding the patient "Brad" can be broken down into three categories: personal, diagnoses-related and information determining service needs. Though there is some overlap for these categories, understanding the
different types of information is beneficial for diagnoses and for determining plans for short and long-term care. The personal information presented includes the fact that Brad is a 32 year
old Caucasian male and at the time of the onset of his illness, he was married with children, employed as a business man and lived in a small, rural community.
After he became ill and was diagnosed, a number of negative factors influenced his personal status, and Brad lost his job, became divorced and lost his financial security, including
his home. The diagnoses-related information collected included the fact that though Brad was generally healthy, he had sustained injuries in a car accident and had a blood transfusion
three years ago, and a number of years after the accident, became ill with pneumonia. Following hospitalization for pneumonia, Brad tested positive for HIV. There are two factors
that should be considered relative to the diagnosis of AIDS for Brad, his positive HIV status and subsequent opportunistic illnesses, and the status of the hospital in the rural communities
(rural communities were slower to put into place screening mechanisms for HIV in the blood supply used for transfusions). Finally, service-related information was also collected. This information
included the fact that Brad is currently homeless, requiring support in a shelter, and the fact that he is currently jobless, requiring support for filing for disability benefits as a
result of his illness. The information collected also supports the belief that Brads current status, his joblessness and homelessness, may be the result of a breech of confidentiality in
