The target is to reduce the current fall incidence rate of 35% to 15% following a year of full implementation and  then to 12% in the following year. To protocol proposed, which is supported by evidence based practice including community based practice, is the use of a multifacotral approach using  established assessment tools. It is recommended that two tools are used; the Hendrich II Fall Risk Model and the DVD environmental check list mode; both of which are already established.  The implementation will take place with elements of the Hendrich II Fall Risk Model used t pre-assess the patients so those at highest risk may be identified and intervention prioritised.  Following a pre assessment and an in home assessment a personalised intervention strategy will be designed by the multidisciplinary care team and implemented. The initial implementation will be for  a three month period with a pilot area, which will be used to optimise the final implementation across the entire organization and aid the change process. The assessment will be  based on statistical analysis of the fall rates, determine first is there has been an improvement, as well as looking at where the improvements have taken place or where there  have been below average improvements.
