In some  cases it may be that aspects of their medical history may not appear important to them so details are not given even if the patient is conscious and lucid.  For many years there have been limited attempts at providing these records for the patient in a limited manner. The use of SOS bracelets or necklaces so that medical staff  can be alerted of an allergy if the patient is unconscious has been around for many years. The knowledge that the dose of a drug or a medical professional being  unaware of a pre-existing condition such as diabetes may result in the exacerbation, and in a extreme circumstance risk the life of the patient. With the development of electronic  record there has been the aim of making medical information transferable so that it can be accessed by those treating the patient as long as they know whos records they  are looking for and how to access them. The next stage from this that avoids the delays even this can cause is for patient to carry some form of medical  record themselves with different fields that can give doctors the immediate patient records they need for effective treatment. With many new devices, from USB pens and devices through to DVDs  there are many choices regarding the way this is presented but more important than ensuring this is a common form of media that can be read is the determination of
