    past, BWH had agreed to affiliation with a competitor MGH and later they would form Partners Health Care System or PCHI. Some changes had been made, most notably in the  cardiac care unit. Yet, Otten wanted to make general changes. He wanted to take the successes seen in the cardiac unit and expand them to the rest of the hospital.  In the past, the changes had been implemented departmentally, but Otten wanted change throughout the organization. The goals for BWH became associated with effectively competing with area hospitals as well  as remaining on the cutting edge of technology. Also, according to the case study, the hospital would have to improve operating efficiency to meet its goals. A network of physicians  had been developed to oversee the goals and implementation of changes. It was learned early on that the culture needed to be changed. At the crux of this case study  is cardiology as this hospital is well known for its excellent cardiac care. One idea is to fully integrate the Cardiac Center with the Cardiac Surgery department. The case ends  by suggesting that the overall goal of the hospital at present is to provide an integrated approach for patients and this must start with the Cardiac unit. In addressing this  issue, several suggestions may be taken into consideration. II. Solutions There are a variety of solutions that may be presented to address the problem at  hand. The problem is not what to do about the situation-that has already been decided-but how to take a large organization such as the one described through a massive transition.  Many people avoid doing things that will bring chaos into their lives and for good reason. Obviously, making the proposed transition will wreak havoc on the existent departments. It could
