    overview of the assessment data tool that will be used to survey the projects target aggregate, the senior citizens at The Erie Center of Health and Aging. Specifically, the  aggregate is comprised of senior citizens aged 65 years old and older with heart disease. Much of the survey depends on self-reported data; another section holds active measurement of  indicator values such as pulse rate and blood pressure. The final section contains recommendations for the individuals taking part in the survey.  Questions Asked The first section of the survey form asks aggregate members for demographic data. Of course these demographic questions include  requests for sex and age, but it also includes questions of height and weight. Other questions in this section solicits information about respondents living arrangements and ethnic background.  It concludes with a request for respondents to describe their overall health. The questions in this section provide demographic data for use in  the study, but it also introduces an educational tool that is used throughout the survey. Attached to those questions that can be seen as being directly related to increased  incidence of heart disease are short statements commenting on the items weight of relative increased risk. It has been long recognized that men have greater risk for coronary heart  disease than women, and this fact is reiterated at the point that the survey asks for the respondents gender.
